At Ambitious about Autism, we're seeking an analytical, organised and resilient Learner Family Transitions Lead to support TreeHouse School leavers to make a successful transition out of our services to meaningful, positive destinations. This will entail working across our services engaging with the trans-disciplinary teams, families and other external agencies, thinking creatively around possible solutions.


To succeed in this key role, our successful candidate will have strong networking and relationship-building abilities skills, plus substantial experience in developing and maintaining referral routes. You should also have:


Degree level education and a qualification in Social Work, SEN, IAG or other relevant area

Excellent communication skills, especially strong presentation capabilities

Impressive interpersonal, time-management and prioritisation abilities

Knowledge and understanding of the EHCP and SEND legal framework, case law and policy environment

A personal commitment to safeguarding and promoting the welfare vulnerable adults

An understanding of the education sector for young people with complex needs

You will:


Build up a network of referral routes across London to support effective transition from TreeHouse School

To lead Transition Reviews for those young people identified as leaving our services

Be fully involved in the Annual Review Cycle, prioritising learners in their final year

Assist learners' parents/carers with information and training to support the transition from TreeHouse School

Develop strong and supportive relationships with the families of the learners

Ensure that learners are meaningfully involved in all decision-making about their future

Provide support at TreeHouse Scool Parent Open Days and Transition events

Develop strong relationships with key external agencies to improve support for learners

TreeHouse School is a non-maintained special school that was founded in 1997. Based in the heart of a thriving community in north London, it educates autistic pupils aged three to 19 from across London and the Home Counties.

Using the Ambitious Approach to education, pupils receive specialist and personalised support through a dedicated transdisciplinary team of qualified teachers, speech therapists, occupational therapists and behaviour specialists.

TreeHouse School works hand in hand with parents and carers, local schools, community groups and employers to maximise opportunities for pupils to enjoy school, succeed in their learning and transition to a fulfilling and rewarding adult life.

Our Curriculum:

Our curriculum is tailored to each pupil’s unique qualities, specific needs, and individual starting points. In this way, we can set accurate targets for progress.

In our individualised learning programmes, we assess each pupil extensively and consider their strengths and what they find more challenging. Our curriculum is creative and flexible, using our natural environment to extend pupils’ learning. You are as likely to see maths being practised in the shop, the community or the garden as you are in the classroom. Where appropriate, our curriculum is linked to and, where necessary, differentiated from the National Curriculum’s requirements.

What is like to learn here:

Pupils at TreeHouse School enjoy many educational trips, including visits to the local shops and cafés, or places of interest in London and further afield. Educational visits are an important part of the vibrant curriculum experience.
