The processes for learning lessons from failures in child protection are not effective, social services inspectors have warned. An increasing number of serious case reviews are taking up “huge amounts” of time and resources with little evidence they improve the protection of children. In its annual report, Care and Social Services Inspectorate Wales (CSSIW) said problems were recurring, despite serious case reviews identifying the same issues “time and again”.
