A useful document to monitor a child’s challenging behaviours in order to identify the nature and potential trigger times, actions etc. This can be used by SENCos/Inclusion Managers as evidence for EHCP applications.

This is a new style monitoring report designed for learners to reflect on their behaviour and progress in lessons. It is uploaded as a powerpoint document so that you can easily adapt it for your own school and learners.
It is absolutely designed for learners to make the biggest contribution to - and reflect on why they are being monitored. I have used this report sparingly and after a detailed conversation with learners who have responded well.
On the back is clear information from the Department for Education - clearly identifying that learners have the right to learn and the sanctions available to schools. You can included your own behaviour for learning agreement underneath.

Aimed primarily at boarding houses, this is a values and choice-based tracking system that promotes reflective behaviour. Students are assigned to family units and the family with the most points every half-term receives a reward. Indiviudal rewards are also available. The element of competition promotes individual responsibility and collective camaraderie.
The core values that are considered essential (found at the top of the sheet), may be amended to suit the environment. The value of the points awarded can be also amended. The premise is that students will make poor choices or “strikes”, and that is part of life, but the expectation is that they must learn from them.Staff enter details as per example. Consequences come into effect after 4, 6, or 8 strikes. The message to students then is that they are deemed not to be learning from their poor choices.
The ladder of consequences can also be amended to suit the particular environment. Ideally, these need to be agreed beforehand and should be considered strong enough to have the desired impact.
Merits and Concerns (or demerits) are linked to academic performance in lessons. These contribute to family totals and the graph is useful for picking up trends. The “Stars” sheet is for a star chart where students can be awarded stars for any positive contribution in house, for example, voluntarily helping to tidy up a room or helping another student in need. Stars contribute to family points totals.
I found a version of this on an American website and adapted it for use in my school. When a child has misbehaved they fill out this form. They have to record what they did but also evaluate why they did it and its consequences. The forms can be filed easily and kept for monitoring.

In this resource, you will find 2 behaviour contracts and a individual behaviour plan.
The first contract is a student led, informal behaviour contract. This is to get students reflecting on their behaviour as a step prior to having a parental meeting. Students can complete this with their teacher, head of year or form tutor and it doesn’t need parents to be present.
The second contract is parent-carer and student behaviour contract. This is a more formal contract which may be used when behaviour becomes more concerning and parent-carer is invited in for a formal meeting before further measures are put in place. This may form part of a behaviour panel and has space to be signed by parents to make the process more formal.
This is detailed behaviour plan which is useful to reflect on triggers and plans for dealing with behaviour. It’s great for sharing with staff who teach the student with poor behaviour.
