SEN One-Page Profile Template

An example of a one-page profile for a (fictional) student with special educational needs. It provides a simple summary of what is important to a student and how they would like to be supported in school. This helps us abide my new SEN legislation by developing more person-centred care and support. As the profile is from the student's perspective is is brightly coloured and in a fun format. This profile is developed via a 1:1 chat with a member of the Learning Support team. The final profile is then issued to relevant staff to inform them of the student's feelings and preferences.

Together for Short Lives is the leading UK charity for all children with life-threatening and life-limiting conditions and all those who support, love and care for them. We support families, professionals and services. In April 2013, we secured Department for Education funding for a two-year project to improve children’s palliative care engagement in the SEND reform (bit.ly/MQOMeH). We are working with children’s hospices and local voluntary providers in each of the 20 SEND pathfinder areas so that: - children and young people with life-limiting and life-threatening conditions are included in Pathfinder testing and benefiting from greater integration - children’s hospice and palliative care providers play an active role in supporting and pathfinders and in the delivery of the reforms - schools and health and social care services are better equipped to support children with life-limiting conditions.

I use this with all three of my SEN children. On a monday morning they come to school and it's on their desk. They then choose reward/treat for Friday afternoon that they will get if they have mostly ticked all sessions on the timetable for the week. It works really well and can easily be adjusted. Their 1:1 are very good at helping them reflect about their work.

The Primary Language RecordFirst devised in the late 1980’s, the Primary Language Record gives teachers a framework for recording their ongoing observations of children’s talking, reading and writing. It is cumulative record of progress in literacy, and its special value is its ‘grassroots’ quality, since it included not only teachers’ and importantly children’s own assessments of their development as readers and writers. It is invaluable both as a long-term assessment of progress and as a basis for immediate forward planning. It’s also very user-friendly.
