This worksheet is to guide learners to understand the requirements of legislation and agreed ways of working to protect the rights of individuals at the end of life, understand factors affecting end-of-life care, understand advance care planning in relation to end of life care, be able to provide support to individuals and key people during end of life care, understand how to address sensitive issues in relation to end-of-life care, understand the role of organisations and support services available to individuals and key people in relation to end-of-life care, by working through the assessment criteria. Learners will also be able to access support for the individual or key people from the wider team, be able to support individuals through the process of dying, be able to take action following the death of individuals and be able to manage own feelings in relation to the dying or death of individuals.
Learners will have the opportunity to learn about the legal and organisational requirements to protect the rights of individuals at the end of life, as well as the key factors that can affect individuals’ end-of-life care. The worksheet has been designed to assist the teacher further plan their lessons. Teachers are encouraged to adapt this worksheet to meet their specific individuals' needs.
