About the role

Vision for Education are delighted to be working in partnership with a highly reputable SEND School in the Stockport area who cater for young people aged 11 - 16 who have suffered Adverse Childhood Experiences (ACEs).  We are looking to recruit a Careers & Transition Lead who will take responsibility for the key transitions in young people’s education.  Using current and targeted information you will offer advice and guidance to pupils and their families.

The successful candidate will ensure every pupil secures an appropriate placement into post-16 or post-18 education, employment and/or training.


Main duties include:

• To lead the completion of transition plans at a meeting where all of those involved in the pupil's transition map out their roles and responsibilities.

• To support the Assistant Head Teachers and Pastoral Leads in the completion and quality assurance of Educational Care and Health Plans, including Enhanced ECHP’s

• To provide training and guidance to all staff on changes in legislation and codes of practise regarding the completion of ECHP’s 

• To plan transitions so that they are gradual, involving taster days and visits to & from employers or educational providers


Requirements for this post of Careers & Transition Lead, include:

• Relevant qualifications to QCF/NVQ Level 3 or equivalent

• Experience writing Educational Health & Care Plans (EHCP’s)

• Comfortable/experience working with vulnerable young people
