An exciting opportunity has arisen for Transitions Officers to join us in each of our regions. This is a unique opportunity to support a wide range of students, some of whom have complex support needs, on the next step of their journey to adulthood, from school to college and then from college to post-college life.  We are looking for experienced practitioners with a good understanding of the wide range of support needs our students may have to join our Integrated Services and Curriculum team.   

This is a strategic role working in collaboration with the Assessments and Placements team, the Curriculum Leadership and Teaching Team and the Integrated Services Team to facilitate well-co-ordinated and smooth transitions from School into College programmes.  

You will have experience of working with a range of students, including those with complex physical and Positive Behaviour Support needs, experience of transitioning students and a good eye for detail with proactive working to ensure that student’s support needs are proactively met.  You will also be an ambassador for OHC and establish good working relationships with external settings such as schools and onward settings.
