This is an inclusion booklet I produced for a 'mockstead'. It outlines my role of SENCO and the SEND practices across school. I have included template examples of all the forms I have created to monitor and assess SEND pupils. it also explains the graduated approach to SEND as well as other things. I have deleted the school logo, names and data. As it stands it would be a valuable template for you to personalise to your school. I use the inclusion booklet for stakeholders, staff training and ensuring consistency across school.

a quick template to enable you to assess the percentage of a cohort at high/medium/low risk of not achieving due to outside influences. This template is filled in as an example to help you, but can easily be cleared for your own use. This is especially useful to show the story behind the headlines of low achieving classes.
