This bundle of assessments includes one of each for Reading and Math, Social Skills, Pre-Vocational (adaptive behavior) and Behavior Observation with Behavior Ratings. Each assessment can be used to develop present levels within a student’s Individualized Education Plan (IEP), Individualized Habilitation Plan (IHP), or 504 service plan. Any or all of them can also be used when describing a student’s academic and or behavior/social skills level to an incoming teacher or therapist. They can be completed by parents, teachers, direct care staff or related service providers (SLTs, OTs, PTs, or ABA therapists). These assessments provide a quick and concise way to gather comprehensive and current academic, behavioral and social information about an individual with low incidence disabilities.
