SEND Target Tracker & Student Voice Pack | SMART Targets & EHCP Outcomes
Make target setting and review a meaningful, ongoing process with the learner, rather than something completed about them at review time.
This 18-page printable pack supports the setting, monitoring and review of short-term SMART targets linked directly to longer-term EHCP outcomes. It provides clear tools for professionals to track progress alongside student-owned pages designed to help learners understand, monitor and contribute to their own targets.
Student voice is central to the resource. Rather than relying on a learner to reflect retrospectively on a whole year when completing annual review ‘My Views’ or equivalent LA paperwork, the student pages can be revisited throughout the review period. This creates a running record of what the learner thinks is going well, what they find difficult, what helps them and how their views develop over time.
Included in the pack:
Target Overview linking short-term SMART targets to longer-term EHCP outcomes across the four broad areas of need
Completed Target Overview example
Individual target review pages for each broad area of need
Student My Target pages supporting meaningful involvement in target setting
My Progress tracker for regular, dated student-led check-ins
One-prompt-at-a-time pages with large open response areas
How to Use guidance
The student pages allow space for a range of communication methods including writing, drawing, symbols, photographs, AAC, eye gaze, pointing and scribing. The individual prompt pages can also be used to reduce visual information, chunk the activity or provide additional space for personalised symbols and other communication supports.
The resource is designed to be flexible rather than prescriptive. Not every learner needs a target in every area and not every learner needs to access or respond to the student pages in the same way.
Targets should be worked on with the student, not simply written about them.
Fewer barriers. More possibilities.
