This 18-page printable pack supports the setting, monitoring and review of short-term SMART targets linked directly to longer-term EHCP outcomes. It provides clear tools for professionals to track progress alongside student-owned pages designed to help learners understand, monitor and contribute to their own targets. Student voice is central to the resource. Rather than relying on a learner to reflect retrospectively on a whole year when completing annual review ‘My Views’ or equivalent LA paperwork, the student pages can be revisited throughout the review period. This creates a running record of what the learner thinks is going well, what they find difficult, what helps them and how their views develop over time. The resource is designed to be flexible rather than prescriptive. Not every learner needs a target in every area and not every learner needs to access or respond to the student pages in the same way.
