Ideas for a guided reading 1/2 hour session to be used daily. The plan suggests a carousel of activities for 5 groups and details each activity. Easily differentiated. (The book review template was borrowed from someone on TES - thanks!)

Transition booklet for year 6 to year 7

(1)
This booklet works through things that year 6 pupils typically worry about when moving to secondary school. The activities each compare primary to secondary. They include: people, the work, timetables, friends, where to keep your 'stuff', places to go to feel safe, what to do when things go wrong (including the incredible 5 point scale, as this was originally written for a learner with a SEN), having different teachers, people who can help, things that are learned, bullying and break times.

EHCP application checklist and evidence gathering

(1)
A handy, easy to use checklist for when applying for an EHCP needs assessment. The checklist allows you to map out what needs to be done as accompanying guidance explains the importance of each document and what will make it effective. I have also attached an example of a chronology of interventions and advice as well as an example provision map and provision proposal.

A lesson plan and carousel of activities to improve persuasive writing skills in year 4. Based on the idea that a part of the school grounds are going to be built upon. Lots of debate and chance for progression through speaking and listening.

Attached are two documents: the SEN audit is a toll that should be completed collaboratively with the school's SEN governor and is a valuable document in allowing you to map out exactly the kind of SEN provision, data, funding, resources etc you have in your school. (Great for brushing up on knowledge and super for Ofsted). Also is an SEN support audit. This looks at pupils on the SEN register who may not have an EHCP and how their needs are being met. Should be used as an observation tool and will easily identify specific and general gaps in school provision.

School inclusion booklet

This booklet, with sections for pupils, parents and space for additional sources of support, can help make clear to children and families the way in which your school supports all pupils at school. It clearly outlines to pupils where they can go for support and what kind of things they might need support with. It also details the changes in school since the commencement of the 2014 SEND Code of Practice.

Pupil Well being group full planning and resources

(0)
This is a plan of 8 sessions for pupil well being. I have planned it for KS2 but could easily be adapted for KS1 or more vulnerable KS3 pupils. I have written them using some ideas from Thrive and Nurture group training and some other, wider reading (some of the happiness resources are from Relax Kids and can be downloaded for free. Everything else I have made myself). Sessions address: helpful and unhelpful thoughts, happiness, being in nature and how this helps us, having a healthy body (exercise), superheroes - celebrating being us, how our friends help us, healthy bodies (diet - cooking, including recipes) and developing imagination and increasing independence.

Resources include a booklet template with HA and LA investigation planning that you can adapt and assemble in booklets personalised to your class. Resource with various materials for parachute making. The project spans over 2-3 1 hour lessons. I carried out this lesson in an observation and achieved 'outstanding&' :)

Documents and guidance for applying for Education Health and Care Plans

Here are three documents that I have used to support an application for statutory assessment. Where I currently teach, we use change trackers as individual education plans. This allows us to complete a full Every Child Matters assessment and highlight areas of need very specifically. The chronology of interventions and advice demonstrates how advice from others has been used and the impact of it.

Easy to access EHCP front sheet including objectives and provision

(0)
This is a sheet that I have created to summarise all of the information found in an EHCP document. I type the objectives and dos and don'ts onto the first sheet and then on the second sheet is a timetable for the week that allows you to map out provision. This has been very useful for holding teachers to account in ensuring that EHCP objectives are being worked towards and allows easy monitoring of progress towards achievement.

Social Story and timetable for Christmas rehearsals

(0)
This is a basic timetable for pupils with additional needs to map out the Christmas rehearsals. Accompanying it is also a social story explaining why we come off timetable at Christmas time. I designed it so that children making use of it take part in only one rehearsal a week, and then participate in a different way for any further The banners underneath the images of the tables in the classroom are to write on the lesson (i.e. English, Maths). I also include images of the teachers who are taking the sessions.

Here is a selection of resources and plans for a WW1 themed fortnight (or more) of guided reading, based almost entirely around Michael Morpugo's Private Peaceful. The resources include a few bits which I have taken from other resources and a link to the bbc website which has Private Peaceful chapter by chapter in audio as well as a teachers pack full of guided reading questions.

Using objectives from the new curriculum, this lesson looks at a story from The Literacy Shed. I have attached the lesson plan and supporting documents. It aims to compare the life of the main character to the lives of the children during the lesson and also includes a short burst writing activity.

Chromatography investigation to be used as part of a CSI type project. A shopping list has been found at the scene of the crime and we can work out which pen was used to write the note. If we know who the pen belongs to, we have a potential suspect!
