This spreadsheet allows staff to track their vulnerable learner meeting notes for each child. This is currently used within a Trust and is widely accepted as a fairly simple and straight forward approach that saves staff time in keeping a track record of notes and how support/concerns develop or progress.
Notes for use:
Data from your MIS is uploaded to the first sheet.
Once the matching child name is entered into column A of the second sheet, it automatically populates the data from the MIS sheet.
Staff simply add notes to the box provided and change the RAG rating.
To create a new meeting, simply copy or duplicate the sheet.
A very simple and effective provision map that includes the option to record and track the cost for each item of provision. You will receive one provision map designed to be used to outline the provision across an entire class or year group, and a separate provision map for an individual student. These are used within a Multi Academy Trust (MAT) and have proved successful to date.
This has been adapted to highlight the phases of SEND outlined in the 2026 SEND White Paper.

These SEND Support Plans have been used within a Primary School setting within a multi-academy trust. This pack contains three templates all similar in design but can also be edited to suit your own needs. These have been designed in a Microsoft Word but can easily be transferred to or opened in Google Docs if required.
Template C has been adapted following the SEND White Paper (2026) which was released earlier this year. These plans have been created with workload in mind and all input can be precise but not overly time consuming.
These are fully editable plans and are simple and straightforward to adapt to your own needs.

Monitoring tracking sheet (monitor which children are currently monitored for SEND and keep a record of those who have previously and their next steps).
Comprehensive intervention tracker sheet (tracks progress of each individual intervention, each child, and provides you an estimated cost for each).
External agency register sheet (keep a record of which children are accessing which services).
Termly SDQ tracking sheet (track children SDQ scores across the year).
Extra-curricular club tracking sheet (track which children are accessing which clubs, how many, and analyse the demographic percentages of children, allowing you to focus in on which children may not be accessing any clubs).
