UPDATED. This is the Excel workbook I've been using and improving on for a few years now. When a copy is left in a shared area on a server, it allows all teaching staff to submit requests on their own individual sheets, and for those requests to be easy to read for technicians on a day by day sheet. It has drop down menus for Date, Classes, Labs and Topic Lists, and is set up to be adaptable with a minimum of editing. README explains how the workbook works, and tips on how to edit it for your own department. Following on from some suggestions and feedback, an updated version is now available.
