This is the Excel workbook I've been using and improving on for a few years now. When a copy is left in a shared area on a server, it allows all teaching staff to submit requests on their own individual sheets, and for those requests to be easy to read for technicians on a day by day sheet.
It has drop down menus for Date, Classes, Labs and Topic Lists, and is set up to be adaptable with a minimum of editing.
