Classroom Library Checkout Sheet (All Grades) & Book Return Label

This library checkout sheet is designed for all grades. On the first page, it has an extra column for the class period which is very helpful when teaching multiple classes especially in middle and high school.

For elementary school or classrooms that have just one class, the second page has the class period column deleted.

There is also a page number spot at the top which will allow multiple pages to be placed on a clipboard in the library.

This is super helpful for me to have everything in one place. In my library, I use a clipboard with several copies of this document and a tub that says Book Return. I have included 2 variations of the Book Return label on the last 2 pages.
