Two sheets for writing comments during guided reading in reception. I printed the documents back to back. One side has the template for recording notes and the other side has the reading expectations from Development Matters for easy reference.
Group guided reading sheet:
For groups of up to 6 children. I would choose between 1 and 3 targets from DM's or sometimes other relevant skills. At the end of the session I would write comments about their reading against these targets. and note whether they were developing, secure or exceeding against each target (in the 1,2,3 boxes)
Individual reading sheets were given to TA's, parent helpers or other adults who read with children individually as a record of this happening.
At the end of each half term/term I was able to use these records towards evidence for progress and achievement during assessment week.
