Behavior Chart for Any Subject - This 2 page weekly behavior chart consists of:
1. One page that tracks class participation, in class work, whether homework was turned in and interactions with others. The categories are rated from 1 to 5 and then totaled at the end of the week. It also has sections for both parent and student comments, plus space for a parent signature. There is a space at the top to write the class for which the chart is being used.
2. One page for summarizing daily behavior for all categories.
There are 2 versions of the behavior chart, one in black and white and one shaded in light purple.
This chart works well for tracking behavior of both regular and special ed students. It is geared more for middle and high school students but could also be used for classroom management of elementary students.
