Three sheets - the quadratic equations, the factorised form, the solution. Lesson idea: I used this as a consolidation lesson with a middle group. Reasonable ability. I photocopied them on to different coloured paper. They then had to match them up. Maybe more effective for KS3, as their is no extension to the activity e.g. x^2 + 4x = 12 or 3x^2 -13x - 10.
