Address Practice: Read It, Write It, Find It: a functional reading and maths set for high school life skills, SEND, and transition programmes. The parts of an address, where they go on an envelope, and how to match a name to a place.

Why I made this: Students are asked for an address on job applications, library cards, medical forms, and packages, and a lot of them stall on the second line. An address has the same parts in the same order every time. This set breaks those parts apart, puts them on an envelope, and then has students use them the way adults do: finding a mailbox, a house on a street, or a name on a delivery list.

What’s included:
• A teacher page: why I made it, how to use both levels, and the skills covered
• Words to know: 10 key words, each with a picture
• 3 pages of practice documents: an addressed envelope with short-form charts, an apartment mailbox panel and a street map with odd and even numbers, and a delivery list with a library card form that has one mistake. All businesses, names, and numbers are made up.
• 12 Level A questions, each with a picture and three answer choices
• The same 12 questions at Level B, with open answers and room to show the maths
• A real-life task: address an envelope from a delivery list and fill in an address form
• An answer key that shows where to find each answer

How I use it: Read through the practice documents together, then give each student the level that fits. Both levels ask the same questions, so the whole class can check answers together. I use the real-life task as a wrap-up or right before a community outing.

Skills covered: naming the parts of an address; reading street and state short forms; placing the return address, stamp, and mailing address on an envelope; finding a unit number on a mailbox panel; using odd and even house numbers; matching a name to an address on a list; checking a form for mistakes and filling one in.

Part of the Functional Life Skills series: every set uses the same layout, so students know the routine when you move to the next set.
