Editable SEN Support Plan and goal-tracking pack for UK schools: SMART targets, weekly data sheets, skills checklists, parent log & review sheets.

Looking for a UK alternative to an IEP? This SEN Support goal-tracking pack puts the whole graduated approach (Assess, Plan, Do, Review) into one organised, printable workbook. It was created by a developmental therapist who works with young children with autism. Every page has clear tables and plenty of writing room, so you can handwrite or type straight into it.

What’s included (6 sections):

  • SEN Support Plan: pupil voice, strengths and interests, baseline, the four SEND Code of Practice areas of need, triggers and calming strategies, and outside agencies (SALT, OT, EP).
  • SMART targets and provision map: record the target, strategies, who delivers it, how often, and the review date and outcome.
  • Weekly data collection sheet: 10 trials per day, total correct, prompt level used (independent, verbal, gesture, model, physical), ABC notes and “what to change next week”.
  • Monthly progress summary: weekly percentages, a blank graph for plotting progress, a progress rating, and a space for the child’s voice.
  • Skills checklists: communication, social, self-help and early academic skills, each rated Not yet / Emerging / With prompts / Independent.
  • Parent/carer communication log, review meeting preparation sheet and review meeting record: everything you need for a calm, well-evidenced review meeting.

Who is it for

SENCos, class teachers, teaching assistants, key workers, early years practitioners, therapists, tutors, and parents/carers supporting a child on SEN Support in Early Years and KS1. It also works as a simple structure for older pupils working at an early developmental level.

How to use it:

Complete the plan at the start of the term. Set up to two or three SMART targets. Use the weekly sheet to collect data, then summarise it monthly. Update the skills checklists each half term. Use the review pages to prepare for and record your meetings.

Formats: printable A4 PDF, plus an editable version (see the file list you upload).

**Please note: **

This pack is a planning and tracking tool. It does not replace your school’s own SEN policy or your local authority’s forms, and it is not an EHCP. Please store any completed pages securely in line with your data protection policy.
