This resource has been designed for students working towards Entry Level 2 Functional Skills English certification (Edexcel-Pearson, NCFE, City and Guilds, NOCN and Open Awards). It contains an editable, age-appropriate, 45-slide PowerPoint lesson and a 26-page workbook on the Entry Level 2 writing composition component - complete a form asking for personal information (e.g. first name, surname, address, postcode, age, date of birth).
PowerPoint Lesson
The PowerPoint clearly demonstrates how to fill in a form with personal details and focuses on the following:

Forename/Christian Name/ First Name
Surname/ Family Name

This 26-page workbook contains 8 information pages, 16 application forms/worksheets and a progress check on filling forms with personal details.
Tasks include:
writing date of birth in different formats
understanding the terminology used on forms - title, surname, forename, Christian name, family name, date of birth, address, postcode
reading instructions - eg. please circle, please tick
completing a variety of forms
