Peer Feedback Forms for Presentations: Middle School (Printable, Any Subject)

Give every student a clear, friendly way to respond to classmates’ presentations. These printable slips use a five-face rating scale and a Star and Wish comment, so feedback stays kind, specific and useful.

What’s included

2 slip designs, 6 slips per page (A4 PDF)
Slip A: rates three areas (Ideas, Voice and clarity, Eye contact) with a five-face scale from “Not yet” to “Excellent”
Slip B: one overall face rating, quicker for younger or newer classes
A Star (something great) and a Wish (something to try next time) on every slip
A one-page teacher guide with a quick-start tip

How it works
Print, cut along the dotted lines and hand one slip to each student per presenter or group. Students circle a face, write a Star and a Wish, then pass the slip back to the presenter.

Perfect for

Class presentations, oral reports and group projects
Any subject or curriculum: English, humanities, science, languages and more
Building listening skills and a supportive feedback habit

The faces are clear vector shapes, so they print crisply on any printer. The slips are white to save ink.

Peer Feedback Forms for Presentations: Secondary School (Printable, Any Subject)

A clean, grown-up peer evaluation slip for secondary students. A four-level scale and specific criteria move feedback beyond “it was good” to comments presenters can act on.
