A clear one-page Teaching Assistant deployment evidence sheet designed for teacher trainees, PGCE students, and early career teachers to record how they effectively used a Teaching Assistant during a lesson.

This template helps trainees gather evidence for teaching standards, lesson observations, and mentor meetings by tracking the type of support provided by the TA and how it impacted pupil independence.

The sheet allows trainees or observers to colour-code different types of TA support, making it easy to visualise how assistance was used during the lesson. The framework follows a continuum of pupil independence, ranging from high independence (self-scaffolding) to more direct intervention (correcting).

The support types included are:

• Self-Scaffolding – pupils work independently while the TA observes and allows thinking time
• Prompting – the TA asks guiding questions to encourage pupils to use their own strategies
• Clueing – hints are given to help pupils recall key knowledge or strategies
• Modelling – the TA demonstrates a process or example for pupils to follow
• Correcting – the TA directly addresses misconceptions or errors

Each level of support is organised into visual, verbal, and written scaffolds, helping trainees identify exactly how support was delivered in the classroom.

The sheet also includes space to record:

• Name, date, class, observer and TA
• Lesson observation context
• Evidence of how TA support promoted pupil independence

This resource is ideal for:

• PGCE / SCITT trainee teachers
• Teaching practice evidence folders
• Mentor observations and feedback
• Professional reflection on TA deployment

The colour-coding approach makes it easy to demonstrate effective TA use and progression towards pupil independence, supporting reflective teaching practice.

File Type: Printable one-page evidence sheet
