This is a marking feedback sheet that I created have successfully implemented in a number of schools.
The sheet includes comments on presentation, positive aspects of work, editable sections to include the LO covered and the grade/level covered. Students can self assess against these objectives, as well as the class teacher.
The second sheet provides space to paste/write questions that students can complete as part of their improvements (EBI's) for weaker areas. There is a section for the teacher and student to comment on the work completed in this section.
