
Easy to use, teacher friendly forms, designed to support teachers and the SENCO to easily identify a pupil’s barriers to learner using a scoring system, as part of the Assess-Plan-Do-Review (APDR) process.
Teachers give each question a score of 0-5 based on how it impacts a pupil’s ability to learn, and the spread sheet with tally the scores. The overview page provides a summary and the drop-down menus allow teachers to select different strategies based on the highest area of need. Teachers are asked to select two strategies (Universal Level/Ordinarily Available Provision) and monitor the impact of this for 6 weeks, then re-score the form. The second stage review allows for the selection of Targeted support from the drop-down menu if there has been little or no impact from the Universal Level strategies. This is an easy to use form, designed to support teacher workload, and be useful supporting evidence for SENCOs to use for onward referrals, including an EHCNA request.
The referral form is differentiated into Key Stages - EYFS, KS1 and KS2, to ensure the questions are targeted and strategies appropriate based on the age and stage of the children being assessed.
