Teacher, Self and Peer-Assessment

I hope this set of this resources will be useful and also hope that it will give teachers and students more opportunities for quick day-to-day assessment with an elegant vibe to it. This resource can be used at Primary level, SEN and Secondary for struggling students and it is not subject orientated. Please add comments on practicality, usefulness and any new suggestions for improvement. This would be very helpful.

The PDF is made of four different assessments: teacher, self, peer and self-assessment for final outcome.
The teacher assessment, in green, has two differentiated forms with the first one (Unit) having more space to describe the progress from 0-5 (each teacher can put their own descriptions based on individual student needs for marking process for each unit of any subject) and on the right-hand side, more space to focuses on Tick- what went well, Target -even better if or Question instead of all of them at the same time.
The bottom bar gives teacher to quickly mark working and learning characteristics such as: not available to engage or with help. The second form is very similar but more for every-day marking: 0 - no effort, 1-minimal effort, 2-sporadic effort, 3-good effort, 4-excellent effort and 5-maximum effort.

The next assessment in red is the self-assessment. This can be used for every day, end of lesson work where individual students can describe their progress, add comments and initiate dialogue with the teacher.

Peer assessment in purple allows other students to comment on their peers progress and, with the help of the word bank, express their opinion and add improvement suggestions.

The final one is another self-assessment, but this time for the final outcome where students can express and describe their personal progress.
