A self evaluation / self assessment sheet for all subjects

This a self-evaluation / self-assessment sheet which I use both in my lessons and in my tutoring where students are able to learn to assess themselves and it is an easy way to keep an eye on progress. It is also an easy summary sheet for parents. I used this during our last OFSTED and the inspector thought it was a good idea for students to self-evaluate / assess themselves.
