I give this to my tutor group at the end of each academic year. They have to fill it and give it back to me. I keep them, and at the end of Y11 I put them all together and give them back to them as a present. They can see how much they have changed (or not!) over the last 5 years of their lives. It is also a great way to do some self-reflection at the end of each year.
