The Foundations for Future Assessment, Evidence & Progress Tracking Toolkit provides educators with a structured but flexible approach to assessing, recording and evidencing progress within personalised SEND, EHCP and Preparation for Adulthood programmes.
Designed particularly with post-16 and personalised provision in mind, the toolkit recognises that progress is not always represented by a qualification grade or completed worksheet.
For some learners, significant progress may mean requiring fewer prompts, communicating a choice more independently, completing a familiar task in a new environment, developing confidence, transferring a skill into the community or taking greater ownership of their learning.

More than simply “achieved” or “not achieved”
A key feature of the toolkit is its focus on how independently a learner can demonstrate a skill.
Rather than simply recording that an activity was completed, practitioners can document the support required and identify progress through reduced prompting, increased confidence, greater consistency and transfer of learning.
For example, a learner who initially completes a shopping task through adult modelling may later complete the same task using visual prompts before eventually completing it independently.
The task itself may not have changed — but the learner has.
The toolkit helps make that progress visible.
