Fractions, Decimals and Percentages Assessment sheet

I created this assessment sheet to use as an initial base line assessment of pupils understanding of Fractions, Decimals and Percentages. On the teacher copy I've included National Curriculum Levels for the skills covered - these range from 3c (3-) to 5c (5-).

I find the assessment useful since it provides formative feedback on the areas that pupils are uncertain of at the beginning of the term. I can then work on these areas and re assess later in the term.
