Here is my version of a quick assessment to gauge the reading level of the student/child. It will provide you with four grade levels. The approximate grade levels will be an independent level (the level the student is comfortable reading), an instructional level (the level the student is currently working at), a frustration grade level (the level which might be a little too difficult for them) and an overall level. This test measures words out of context so the student cannot pick up on the meaning of words by using potential clues from the rest of the sentence (context clues). These clues sometimes help the reader to determine the meaning of unknown words or determine what the words are. This test consists of 15 lists of 20 words each for the grades K-12 for a total of 300 words. There are 3 lists for kindergarten and then 1 list for each grade afterward up until grade 12. I have tried to make sure the lists are equally difficult. The words are provided out of context because weaker readers tend to rely on how the word is used to identify the word as well as its meaning. There is a checklist for the teacher/parent in this bundle so you can check off which words are read correctly and incorrectly. You have the student start with list 1 and then continue reading through the lists to determine their independent, instructional, frustration and overall reading levels.
