The Student Behaviour & Mental Health Risk Screener is a teacher-rated, non-diagnostic Tier 1 PBIS/MTSS screening tool to identify student needs.

This practical screener helps educators quickly spot internalizing and externalizing risks to plan targeted Tier 1 and Tier 2 interventions.

What’s Included:

16 Screening Items: 8 Externalising (behavioural) and 8 Internalising (mental wellness/emotions) questions.

Classroom-Wide Sheet (Google Sheets): Screen up to 35 students on a single tab with automated color-coded formulas that sort students into support Tiers.

Individual Student Form (Google Forms): Quick individual submission option for targeted staff concerns.

Intervention Guide: Built-in list of recommended Tier 2 interventions.

Likert Rating Scale: Standard 0–3 scoring system (0 = Not observed, 1 = Occasionally, 2 = Frequently, 3 = Almost always).

How to Use:

Option 1 (Google Sheets - Recommended): Best for whole-class or school-wide universal screening. Automatically calculates risk levels and highlights students who need extra social-emotional or behavioural support.

Option 2 (Google Forms): Best when an individual teacher or support staff member has concerns about a specific student.

Note: This resource is a non-clinical, non-diagnostic screening tool designed to guide school support teams and intervention planning. Includes permission to use across your school/team.
