SEN Cause for Concern Flowchart – Pre-EHCP Process

This easy-to-follow flowchart has been designed to support SENDCos and school staff in understanding and implementing the processes involved prior to applying for an Education, Health and Care Plan (EHCP).

The flowchart begins at the point where a potential Special Educational Need or Disability (SEND) is identified — whether by a parent, teacher, or outside agency — and clearly outlines the steps, documentation, and decision-making processes that should follow.
