As part of my training to become a SENCO i have developed a new way for recording and tracking progress for children with SEN. This has now turned into a Learning Diary. The first step is to fill in the one page profile with the child and agree on some targets. Then we use the parent interview form to include parents views and opinions in the process. Once targets are agreed they are entered into the target proforma. These targets do not have a set review date as they will continue to evolve and change. We then use the learning log as a way of evidencing progress and interventions. Once a child has complete a target they will receive a certificate which outlines the target achieved and sets out a new target. This is a way of keeping parents in the loop. The SENCO will review each child's personal learning diary at the end of the term and conduct an interview.
Have also included a Power Point presentation used in a staff development meeting.
