SEND Developmental Tracker

Developed based on the Department of Educations, EYFS SEND Assessment Tools (https://help-for-early-years-providers.education.gov.uk/support-for-practitioners/send-assessment) this spreadsheet is a user friendly way to track children’s progress if they are working significantly below Reception age expectations. This tracker allows children to be tracked based on the four broad areas of need, and automatically calculates a score to show children’s developmental progress on a termly basis. The spreadsheet allows for the selection of a termly target and short review of the child’s progress as part of the Assess Plan Do Review (APDR) process. The overview page provides a summary of the children’s progress and can serve as a simplified One Page Profile.

This is a help tracker to be used as evidence for onwards professional referrals by the SENCO, including as part of an EHCNA request.
