Helping Every Child Feel Heard, Safe and Included

Give pupils a voice and help them share how they feel about school with this SEND Pupil Voice Questionnaire.

Designed to gather children’s views and experiences, this questionnaire provides an opportunity for pupils to reflect on their happiness, safety, learning and sense of belonging within school.

Using a simple 1–5 rating scale, pupils can express how much they agree or disagree with each statement, helping adults understand what is going well and where additional support may be needed.

What’s Included in the questions?

The questionnaire features 15 statements covering key areas of school life:

Happiness and wellbeing – feeling happy and supported at school.

Safety and safeguarding – feeling safe and knowing how to stay safe online.

Trusted adults – knowing who to approach for help or when feeling worried.

Friendships and relationships – exploring kindness between children.

Learning and teaching – understanding lessons, enjoying learning and receiving help when work is difficult.

Inclusion and participation – feeling included in lessons, clubs, trips and activities.

Fairness and support – understanding whether adults treat children fairly.

Breaktimes and lunchtimes – feeling safe and happy during less structured parts of the school day.

School belonging – feeling proud to be part of the school.

The questionnaire also includes space for the pupil’s name, year group, term and additional comments.
