A toileting and changing record that works for the whole class.
One sheet covers a student who is using the toilet independently, a student who needs support or reminders, and a student who is being changed. Nobody’s sheet announces which.
How it works
One line for each visit or change: date, time, what happened, how the student let staff know, the support given, a note if needed, and initials.
Two short keys keep entries quick. What happened uses T for used the toilet, D for dry, W for wet, BM for bowel movement and CC for change of clothing. Support given uses I for independent, V for visual or symbol, P for prompt or reminder and A for adult support.
Two columns worth having
How they let us know records whether the student used words, a sign, a symbol, AAC or a gesture. For many students that is the actual target being worked on, and it is easy to lose if the sheet only records outcomes.
Support given shows the level of support at each visit, so a move from adult support to a visual prompt is visible on the page rather than being remembered at a review.

Clear print sizing, generous writing space and clean greyscale printing, so it holds up on a clipboard and photocopies well.
The support and communication columns feed naturally into the review section of a student support plan, and into any independence or communication targets already being worked on.
