Multiple choice quizzes - Template and some samples

Here are few samples and a template that I use to create multiple choice quizzes for my classes. I get all of the questions from DiagnosticQuestions.com (don’t worry - I got permission to upload some here!). DQ is a fantastic free website and can be used interactively by your students so the questions are marked for you and you get feedback on your classes misconceptions too.
