I have made some labels for my class that I would use. They correspond to the new Areas of learning. Feel free to edit or add more. I have kept them colour coded but these can be changed too.
Obviously all areas are cross curricular but I have stuck to the main themes.

For those of you who love tracking, documenting, and keeping files, this weekly behavior management tracking system is for you! This system has been made for the Green, Yellow and Red systems that are already in place in your classroom, but can be adjusted easily to any other 3-Tier systems by editing the category labels (its a Word file). This system is meant to complement your existing system, but offer a way of tracking and documenting.
At the end of each day, I would have a student go down the list and put colored checks for each student, so by the end of the week I had a full week's worth of data on behavior with specific codes annotated next to each day when points were taken away. This made referencing and phone calls home easy and data based.

Check out this simple planner that students can use to track their assignments throughout the week! Students will list their various assignments and there is a convenient box beside each component that they can check off as they finish. This will allow student to see the big picture and both plan ahead as well as catch up if needed.
This will also help teachers to assign work ahead of time for students who like to get ahead or for students who will be absent. There is also an added section for "Other To Do" items.
The tracker is listed side by side and has two identical documents so you get four full weeks of assignments on each one each sheet of paper.
