- 30 questions related to a student's personal study habits, to be answered by highlighting an option among 'hardly ever,' 'sometimes,' 'usually,' and 'most always.'

- 3 reflective writing prompts designed to have students explain and analyze their personal study habits, challenges in the process of studying, approach to learning in the classroom, and engagement with course materials (with an emphasis on SQ3R) in paragraph form.

- A "Study Distractions Analysis" worksheet featuring 12 true-or-false questions about the three most common places where individual students study.
