Here is an example of a behaviour chart for children who need to see visually what behaviours are acceptable, what are becoming unacceptable and what are a definite no. It can be adapted to suit the needs of the child/children but will be a good starting point. Talk through it with the child and get these children to pick the consequences and rewards with your agreement.

Academic Student Contract and Weekly Monitoring Sheet

There are two documents in this packet: The academic student contract sheet helps put in place strategies to get the student to improve their grades, but also lets them know that they are in danger of failing if they do not turn their academic performance around. The daily motoring sheet is a tool to help parents, guidance counselors and teachers see what is going on in all of the students classes daily. The sheet is set up to record a whole week of what the student has done and has not done. The documents are in Microsoft Word and can be easily edited to meet the needs of individual schools.

I created this resource when I was completing my teacher training as I wanted to evidence that I had explicitly planned for a teaching assistant. I found it worked really well, especially as the TA was fairly new to the role. I would recommend this resource to NQTs and teachers with new teaching assistants. I have left an example plan that can be edited. The second sheet also allows the TA to give some written feedback about how the activity went.
