This is a weekly behaviour chart template that I use with my children with behavioural difficulties.

I put their name on it (so they have ownership) and the week's date.

The table is broken down into sessions for each day. For each session that the child completes his targets such as: complete his/her work; not disturb others; speak nicely to others etc, they get a stamp. This could be a sticker, a smiley face or a coloured stamp saying well done.

You set a target for them to get X amount of smiles or stamps a day, if they do it then they get a reward or the chance to do something they like to do.

If the child does not have a good session then I give either a straight or a sad face. I would then discuss with the child what went wrong in the session and how we can move forward.

I also add pictures to the chart of things the child is interested in like cars etc. You can also print on different coloured paper to make it interesting.
