Baseline Assessment Summary Sheet

This resource is to fully assess students who will have an Education, health and care plan (EHCP) when they join a new school. It assesses all aspects of a students well being and then fragments them in into micro assessments so that progress a students makes in all areas of their school life can be assessed. This includes personal care, mobility, communication and behaviour. This assessment can then be re-assessed year on year against the initial baseline to demonstrate progress against the baseline and the students entry into a new education establishment. Always useful when Ofsetd or the LEA needs evidence of student progress.
